Amendments to the
Florida Rules of Civil Procedure
Supreme Court rules
opinion SC08-1141: Amendments to the Florida
Rules of Civil Procedure requires that starting January 1st,
2010, an amended Civil Cover Sheet, Form 1.997 and the new Family

In addition, as part
of the rule amendments, the opinion adopts a revised Form 1.998
Final Disposition Form.
These forms
significantly redefine the categories under which civil and family
cases must be reported by the Clerk of the Circuit Court.
Please be advised that
failure to submit the proper Cover Sheet will result in summonses
not being issued until the proper Cover Sheet is filed.
